Here's all of the information your bridesmaids will need to place their order with us. Please note that some of the forms are REQUIRED forms – these MUST be completed and returned to us for EACH of your bridesmaids. Just follow the step by step instructions below:

PLEASE NOTE: We ship 10 weeks from when ALL of the completed information is received for the ENTIRE bridal party.
Step 1. What Anna Elyse Needs From You
This form tells us all about you and your bridal party. It's important information so please fill out the form completely and return it to us. This completed form helps us to help you throughout the process. This form is required ONLY from the bride.
Step 2. The "How To" on Sizing
These are the "in's and out's" on getting measured properly and choosing the correct size. Ladies, this is a MUST READ!
Step 3. Anna Elyse Measurement Form
Follow the simple but important measurement instructions. This form is to be completed by and for EACH girl ordering. Style name, color and size MUST be included. REQUIRED form.
Step 4. Anna Elyse Fit Guides
These guides are for your reference to assist in choosing the correct size. There is a fit guide for you to review for the particular style you are choosing. Be sure to check out the "Helpful Hints" for guidance in choosing the style and sizing that is right for you and your body type. If you fall between sizes or don't quite fit into one size clearly, please don't hesitate to call and consult with us. While we cannot choose your size for you, we will be happy to work with you to be sure you are comfortable with your choice.
Step 5. Anna Elyse Order Form
Ladies, there is no way around this one. This form MUST be filled out COMPLETELY including size, color and style name and signed by the person choosing the dress. PLEASE NOTE: Orders may NOT be called in to us!! We need it in writing. REQUIRED form.
Step 6. Policy and Contract Form
Individual Policy and Contract forms MUST be completed by each member of the bridal party and returned with their order. REQUIRED form.
Step 7. Payment Method Form
A deposit of $150 for short dresses, $200 for long dresses or 50% for Bridal Gowns is required with your order. Any balance is to be paid prior to when the dresses ship. If paying by Credit Card, the same card will automatically be charged for your final payment. A Sales Order will be sent noting the order total, any deposit paid, the balance due (excluding shipping and any applicable sales tax) and ship date. Your balance will be charged one day prior to the ship date. REQUIRED form.
